Social determinants of health
ASAPs provide a comprehensive set of social and health care services, and are best positioned to ensure that contracting organizations are responsive to the social needs of their plan members, including nutrition, financial management, housing, and transportation. We conduct intake and referral to determine an individual’s social needs and connect them with resources where needed.
