Keith Dzialo

As Care Coordinate’s Head of Integration and Support Services, Keith Dzialo, has the responsibility of managing the integration and support needs of various network-based initiatives. Keith brings over 12 years of leadership experience in provider operations and project integration within Massachusetts’ Home and Community-Based (HCBS) environment.

Before joining Care Coordinate, Keith was a Systems Integration Manager for the Personal Care Line of Amedisys, one of the nation’s leading providers of Home Health, Hospice and Personal Care. In this role, Keith was charged with deploying a Connected Caregiver/Electronic Visit Verification (EVV) platform to over 2,000 caregivers across 3 states. He served as a subject matter expert supporting the State of Massachusetts’ EVV initiative by working closely with EOHS, EOEA and State selected data aggregators.

Additionally, Keith played an integral role in the successful integration of multiple Personal Care acquisitions during his tenure with Amedisys.

Keith got his start in Home and Community-Based Services managing operations for Affinity Home Health Care, his family-owned business. The Affinity Home Health Care operation was charged with delivering quality home-based care to thousands of elders throughout communities on the North and South Shore of Massachusetts annually.
