Care Coordinate is putting our network of community-based services on the map, as a central entity navigating the complexities of coverage and care.
As our name suggests, Care Coordinate serves as a centralized administrative hub for Massachusetts Home Care (MHC) and the Aging Services Access Points (ASAPs), our network of 24 nonprofit member agencies reaching every corner of Massachusetts, whose mission is to support older adults and individuals with disabilities to maintain their independence and live with dignity in their communities.
Care Coordinate is the convergence of high-quality services and efficient administration.
- We collaborate with organizations such as health plans, accountable care
organizations, long-term care systems, and community-based service
providers. - We are a central point of connection for our member ASAPs. Our administrative
architecture includes contract management, prior authorization, and
systems integration for consumer-centered care management and
coordination. - We leverage industry best practices and eliminate costly inefficiencies while
connecting consumers with core services – long-term services and supports
(LTSS) and community-based, culturally competent care.
